Use this service to decline an invitation for a health review or vaccination.

If you want to attend your review but do not want to do specific parts of it, you can let us know in the form.

You can use this service if you:

• are registered at the surgery
• have received an invitation

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• details of the health review or vaccination you want to decline
